Please bring your insurance card(s) on the day of your appointment so that the correct information about you can be transmitted to the insurance company. Also, we will need your social security number and employer of the insurance policy holder.

Some insurance plans require pre-certification of certain exams prior to you receiving radiologic services. If you are unsure of the requirements of your insurance plan, it is suggested that you call the “800” number listed on the back of your identification card or refer to the insurance plan handbook provided by your employer. Your referring physician usually knows which imaging exams require pre-certification. In general, your referring physician will obtain the pre-certification if it is necessary. Failure to comply with your insurance providers requirements may result in a denial of benefits. You would then be responsible for the full charge of the imaging exam.

By contractual obligation, we participate with and accept payment from the insurance companies listed below. This means that we accept as payment in full, the amount your insurance company pays us for our services. You should understand, however, that you are responsible for paying any deductibles, co-insurance (co-pays) and non-covered services.

If we do not participate with your insurance plan, your plan may still allow you to obtain an imaging exam from SCIC as an out-of-network provider. You should understand that we would bill your insurance company for services rendered but you will still be responsible for paying any balance on your account after reimbursement is received from your insurance company as well as for paying any deductibles.

Billing statements are mailed to patients on a monthly basis. For your convenience we accept cash, check, Visa and Mastercard for payment.
